Actual work would identify the inspection date, responsible professional, standard, agreed systems, documents reviewed, interview sources and intended users. Here, maintenance histories and test records are assumed unavailable to demonstrate how gaps are recorded.
3. Executive summary
Priority attention centers on a possible moisture issue, drainage near a walkway and access needed for further evaluation. No diagnosis or cost conclusion is supported by this fictional record. Unknown conditions stay unknown until appropriate evidence is obtained.

5. Evidence and records
A real report links observations to dated, location-specific photographs and identified records. This demonstration contains no inspection photographs. An example is not evidence of an actual deficiency or of MeridianVault completing an assignment.
6. Cost and planning boundaries
No repair quantities or prices are established here. A cost-bearing assessment needs an agreed scope, defensible quantities, cost sources, date, assumptions and planning horizon. For an ASTM PCA, document review, interviews and applicable cost opinions form part of the assessment process.

For the roof item: “repair reported” would record a client or contractor statement. “Evidence received” would identify the submitted record. A follow-up visit and professional review would be separately documented before a conclusion changes. An upload alone does not verify repair.
8. Limitations and use
Actual inspection reports describe sampling, access, weather and scope limitations. An inspection is not exhaustive and does not guarantee future performance. Engineering, environmental assessment, code certification and other specialist work require separate qualification and scope. This fictional example has no signature and cannot support a transaction or funding decision.
